Lafayette School District Volunteer Requirements

Do you want to volunteer at school?
Read to your child's class?
Drive on a field trip?
Then you MUST complete the below requirements before January.

This year the Lafayette School District is updating the requirements for on-campus volunteering. These involve some safety and health screenings (see below for details). We understand that these requirements will take some time and commitment on your part but they are vital in keeping our Bobcats safe and healthy!

Depending on how frequently you plan to volunteer, there are 3 tiers of requirements. Please find the appropriate tier for you and review their requirements below.

    

These requirements need to be completed by January 2, 2024 to be permitted to volunteer at the school.

(Please note that each school has its own unique form (but same requirements); families with children in more than one school will need to complete the volunteer form for each school.)


We hope to keep the Burton Valley campus FULL of volunteers!
Please help us do so by completing these requirements.


Tier 1 Volunteering:

Required for all one-time Volunteers: less than 8 hours

STEP ONE:

Acknowledge that you have watched the FERPA training video and agree to a search by school office staff of the sex offender registry (Megan’s Law).

COMPLETE TIER 1 FORM NOW 


Tier 2 Volunteering:

Required for all frequent Volunteers: more than 8 hours

STEP ONE (same as tier 1):

Acknowledge that you have watched the FERPA TRAINING VIDEO and agree to a search by school office staff of the sex offender registry (Megan’s Law).

STEP TWO:

Tuberculosis Screening Requirements

This TB Screening will ensure that we are adhering to the most recent guidance from the California Department of Public Health.

1. You must receive a TB Screen by a healthcare provider. This screening involves 5 simple questions followed by a signature from your healthcare provider. The results of this screening may result in the need for TB skin test or blood test if you have risk factors.

2. Completed forms need to be uploaded at link below and are valid for 4 years.

 

Where can I get a TB Screening?

• Contact your Dr - many providers will complete the form via a quick telehealth appointment or by just messaging them the form and answering a few questions.

• Various CVS Minute Clinics offer TB Screening Appointments online.

• Most providers provide their own TB Screening form OR you can ask them to fill out this one.


ONCE YOU'VE COMPLETED THE ABOVE STEPS, PLEASE FILL OUT AND UPLOAD YOUR FORMS AT THE LINK BELOW.

COMPLETE TIER 2 FORM NOW 


Tier 3 Volunteering:

Required for Volunteers that are Driving and/or acting as an Overnight Chaperone for a School Field Trip

STEP ONE (SAME AS TIER 1):

STEP TWO (SAME AS TIER 2):

STEP THREE:

Show Proof of Drivers License and Declaration of Drivers Insurance

PLEASE NOTE: The Declaration of Drivers Insurance is NOT your card. It is a page on your policy that shows that you are covered for a certain dollar amount for Bodily, Property and Medical.

Images of both need to be uploaded to the form below.

ONCE YOu've COMPLETED the above steps, PLEASE fill out and upload your FORMs at the link below.

But wait, there's one more step for Tier 3!!!!!

STEP FOUR:

Fingerprinting

All volunteers transporting students or chaperoning overnight field trips must pass a Live Scan background check.

Option 1: Make an appointment at the District Office

• Follow these instructions to make an appointment

• Fill out this form and bring to your appointment along with ID. Note: You must bring this exact form to your appointment. The appointment costs $60.

• Please allow 3-4 weeks for your results to be processed. Your results will be automatically sent to the District. No other action is needed.

 

Option 2: Make an appointment at an alternative Live Scan location

• Live Scan locations in Contra Costa County are listed here.

• Fill out THIS FORM and bring to your appointment along with ID. Note: You must bring this exact form to your appointment. Appointment costs vary on location.

• Please allow 3-4 weeks for your results to be processed. Your results will be automatically sent to the District. No other action is needed.
