Lafayette School District Volunteer Requirements
New for the 24/25 School Year!
Many of these requirements take several weeks to complete so please start NOW!

For the 2024-2025 school year, the District will use "Be A Mentor," an online portal to streamline volunteer screening. For all volunteer roles at BVE you will be required to have TB Screening Clearance AND Live Scan Fingerprinting next year.
These requirements need to be completed several weeks before you wish to volunteer to be permitted to volunteer on campus or to chaperone a field trip.
Step 1: Create A 'Be A Mentor' Account
All volunteers must register on the 'Be A Mentor' online portal. This system will streamline the registration process for parents and caregivers, making it easier for them to serve in all volunteer capacities.
CREATE AN ACCOUNT NOW
When you complete the volunteer requirements below, you will upload the appropriate documentation to your 'Be A Mentor' profile.
Step 2: Get Fingerprinted via Live Scan
All volunteers (even one-time volunteers) must pass a Live Scan background check. It can take several weeks for your results to come in so please get this done ASAP.
If You Did Not Get Live Scan FingerPrinted Last Year:
• Find a local Live Scan provider. Many UPS Stores allow for walk-ins, please call before going.
• Fill out THIS FORM and bring to your appointment along with ID. Note: You must bring this exact form to your appointment. Depending on location, the appointment may cost $60-100.
• Please allow 3-4 weeks for your results to be processed. Your results will be automatically sent to the District. No other action is needed.
IF YOU DID GET LIVE SCAN FINGERPRINTED LAST YEAR:
• If you completed Live Scan for LAFSD in the 2023-24 school year, repeating this step is optional if you create your Be A Mentor profile before August 31, 2024. The district will auto-upload your Live Scan results from last year, no other action is needed.
Step 3: TB (Tuberculosis) Screening Clearance
All volunteers must show proof of TB clearance within the last four years.
IF YOU DID NOT SUBMIT A TB CLEARANCE LAST YEAR:
• You must receive a TB Screen by a healthcare provider. This screening involves 5 simple questions followed by a signature from your healthcare provider. The results of this screening may result in the need for TB skin test or blood test if you have risk factors.
• You can contact your own healthcare provider or find community testing options.
• Most providers provide their own TB Screening form OR you can ask them to fill out this one.
IF YOU DID SUBMIT A TB CLEARANCE LAST YEAR:
• Simply upload last year's form to the 'Be A Mentor' portal! Your results are valid for 4 years
ONCE YOU'VE COMPLETED THE ABOVE STEPS, PLEASE UPLOAD YOUR COMPLETED FORM TO YOUR 'BE A MENTOR' ACCOUNT

Step 4: Upload Drivers License and Declaration of Insurance
**Only Required for Volunteers that are Driving and/or acting as an Overnight Chaperone for a School Field Trip
• Simply upload your Drivers License and Declaration of Insurance to the 'Be A Mentor' portal!
PLEASE NOTE: The Declaration of Drivers Insurance is NOT your card. It is a page on your policy that shows The name of your insurance company, Your name as an insured driver and Your policy expiration date.
